Purpose & Discipline Coaching Program Contract

Coaching Services – Mens Lifeline Academy, LLC offers coaching services that aims at encouraging, uplifting men to foster a life of purpose and to build a lifestyle of discipline. The program is designed to teach men how to develop a growth mindset and to move away from right v. wrong thinking patterns by taking on a philosophy of learning to understand how their actions influence the outcome of the desired goal. One pathway of reducing the shame/guilt that plagues men is seeing how their actions led them closer to the goal by being effective and/or away from the goal by being ineffective.

Participant Agreement – The participant in the Purpose & Discipline Coaching Program agrees to pay for the services provided in full and/or partial payments to be completed within the month. The participant agrees to attend all scheduled appointments on time, and the appointment can be re-scheduled with a 24-hour notice. The coach is allotted to cancel the appointment if the participant does not arrive within a 20-minute window. In addition, the participant agrees to arrive on time, be open/honest, complete assignments and be coachable.

Format Agreement – The participant will be seen weekly for one month for a duration of 45-60 minutes. The individual agrees to complete the assigned tasks assigned by the coach and be prepared to have an open/honest dialogue to be effective in reaching the participant’s desired goal. Group coaching services will be available for participants willing to attend in a group format with meeting weekly for 45-90 minutes in duration for one month. The Coach agrees to meeting on time and to provide guidance, accountability/support as well as strategies that tailored to the participant’s SMART Goals by maintaining professionalism and confidentiality.

Confidentiality – All personal information shared by the participant is confidential unless, they have provided consent to share information on social media platforms, information required by law and/or court order and/or provides a danger to themselves/others. In event of group coaching confidential is expected amongst all the participants. Corrective actions may take place in the event information in the group is shared outside the group.

Termination Agreement – The participant may terminate the agreement with written notice and if the participants terminate early without written notice, maybe subject to no refunds.

Liability Waiver – The participant agrees to hold the coach harmless from any claims, damages, or losses arising from participation in the program. In addition, the participant understands that results depend on personal effort, actions and follow through.
